2024 has been still another excellent year for IFIC’s EU funded projects, during which we have successfully completed several projects and started new ones. Overall, IFIC is proud to have participated in 25 EU funded projects, contributing to the advance of people centred, continuous, coordinated care in its different projects. 2024 was a rewarding year for our EU projects communication activities, with successful project completions from UNICA4EU, CANCERLESS, VALUECARE, and the continued progression of INCAREHEART, CAREMATRIX, DigiCare4You, ROSIA, CO-CAPTAIN, together with LAUREL project starting in 2024. These projects present challenges that we embrace, and this year demonstrated our ability to apply the lessons and experiences gained from previous projects as we take on new ones.

Additionally, IFIC has large expertise in leading dissemination and communication work packages, having recently considered as an excellent example by the reviewers of one of the closing projects. Due to IFIC’s considerable network and ability to connect with a wide range of stakeholders, IFIC has been a key player in several projects for making the projects and their results visible and understandable for different profiles of people. We are proud of the contributions that we have provided to the EU funded projects, described below.
INCAREHEART is a Pre-Commercial Procurement (PCP) project, that has been ongoing since 2021. The challenge to the market has been to develop digital tools that enable integrated care for people living with chronic heart failure in the EU. IFIC has contributed to the codesign, change management, capacity building and dissemination, having led the synergies with other three PCPs to join forces for dissemination and communication.
CAREMATRIX is a PCP whose main goal has been to request to the market of innovative integrated care technologies to improve the management of multimorbidity. Being in its last phase of implementation, IFIC has been supporting the project in the unmet needs’ identification, organisation of the Open Market Consultation, stakeholder mapping and networking with other EU funded projects. IFIC has led on dissemination and communication and supported exploitation activities.
ROSIA, a Pre-Commercial Procurement project launched in 2021, focuses on developing a telerehabilitation platform. IFIC has supported the identification and monitoring of KPIs for evaluating its third phase and contributed to communication and dissemination efforts.
VALUECARE, a five-year Horizon 2020 project ending in 2024, aimed at integrated care for older adults. IFIC led communication, participant recruitment, and co-authored chapters for the VALUECARE book, earning praise from the European Commission.
CO-CAPTAIN, a Horizon Europe initiative, addressed cancer prevention for individuals with mental health conditions. In 2024, IFIC co-designed a patient navigation model and supported the Health Inequalities group, establishing an international community of ambassadors to facilitate the dissemination.
CANCERLESS, a Horizon 2020 project, focused on cancer prevention for people experiencing homelessness. IFIC developed a cancer care transformation blueprint, authored key publications in high-ranked journals, and presented results at high-profile conferences.
DIGICARE4YOU, a Horizon 2020 pilot, aims to prevent hypertension and diabetes in children and families through community-based interventions. IFIC co-leads dissemination efforts. IFIC has supported the consortium by addressing health inequalities during the intervention phase, ensuring equitable access and outcomes.
UNICA4EU was a pilot project focused on mapping the current landscape of Artificial Intelligence (AI) applications for Childhood Cancer to understand how AI could help improve care pathways in the EU. IFIC supported the identification and assessment of data along the patient journey, leading on the impact analysis, based on mixed methods research including interviews with key experts on AI health devices roadmap to implementation, identifying its challenges and facilitators.
LAUREL started in 2024, being its final goal to advance actionable policies for integrated long-term care in the EU. IFIC has led the initial literature review, involving the identification of current frameworks for integrated long-term care, and is also leading on dissemination and communication WP. IFIC has been supporting the networking activities, in close collaboration with other consortium members.
