Integrated care for older people with frailty has a growing evidence base. This webinar compared and contrasted three very different services, all making a unique contribution to care closer to home: a Hospital at Home and intermediate care team from Barcelona, an early supported discharge and community stroke rehabilitation service from Hawke’s Bay, New Zealand, and a day hospital based rehabilitation service from SE Sydney, Australia.
