Ageing and Frailty SIG
Knowledge Exchange
On 22 June 2026, IFIC Scotland's Ageing and Frailty Special Interest Group, in collaboration with the University of the West of Scotland, hosted an international Knowledge Exchange bringing together experts from Australia, Scotland, Canada and Catalonia to share research, innovation and practical examples of integrated approaches to healthy ageing and frailty.
The event showcased a diverse range of initiatives supporting older people to live healthier, more independent lives. Presentations explored nurse-led models of care for older adults with frailty, national improvement programmes, community-led approaches to ageing well at home, integrated care programmes for people with complex chronic conditions, and regional transformation projects designed to deliver more personalised, coordinated care closer to home. Speakers also highlighted new educational opportunities to strengthen knowledge and capability around frailty across the workforce.
The Knowledge Exchange provided an opportunity for participants to learn from international experience, explore common challenges and identify practical approaches to improving integrated care for older people. The discussions highlighted the value of collaboration across countries and sectors in advancing person-centred care for ageing populations.
