Integrated Care Solutions© is a service that helps support IFIC’s international clients in the effective design and adoption of their integrated care programmes. We achieve this by bringing together the benefits of IFIC’s own content experts with our growing global network of members, associates and knowledge partners. By working with our clients to understand their aspirations for integrated care in their own context, Integrated Care Solutions© provides tailored support that help to advance strategic thinking and translating that into implementation, enabling sustainable service transformation at local, regional and national levels.

Integrated Care Solutions© provides customised services that combine research, best practice and expertise to design and support the effective and sustainable implementation of integrated care.

Evidence-based domains that underpin successful Integrated Care (Europe) Thursday, 12 July 12:00pm – 13:00pm (GMT+1)

Evidence-based domains that underpin successful Integrated Care (Asia Pacific) Friday, 13 July 08:00am – 09:00am (AEST)

This webinar explores the evidence-based domains that underpin successful integrated care systems and presents a framework and toolkit called the Accountable Integrated Care Health Check that enables health and care systems to understand where they are on their journey of integration.

The Health Check comprises 10 domains which are used to gather and analyse evidence using range of methods including document review, analysis of unstructured data, surveys, 1-1 semi-structured interviews, focus groups and observation. The Health Check is a prospective, learning exercise that is co-delivered with the system in question and intended to inform on ongoing programme of integration development.

The webinar shares some of the patterns that have emerged across a number of health and care systems and what this tells us about the state of integrated care today and priorities for future action.
