Exploration of the capabilities that enable the human level transformation necessary for collective action, sharing power, and fostering the conditions that improve the delivery and experience of health and social care. This theme focuses on demonstrating the pillars of integrated care related to shared values, vision, system wide governance and leadership.

Exploration into how health, wellbeing and experiences are improved with people and community. To improve population health and achieve health equity, there is a need to address the multifaceted factors that contribute to health disparities and distrust in the health care system. To enable better outcomes, many systems are re-orienting their focus to community, working in partnership, and building trust. Improvement that is co-designed through the connection of people, providers, communities, and systems. From advancing health and wellbeing through Asset Based Community Development, Indigenous Ways of Knowing and Being, cultural sensitivity and meaningful partnerships with patients, caregivers, and communities. This theme focuses on demonstrating the pillars of integrated care people as partners in care, population needs and local context, and resilient communities and new alliances.

Exploration of the complexities to building emergent care teams, integrated systems and impact of workforce dynamics, roles, and digital health applications. Considering how systems and teams are designed and organized to optimize the delivery of care and activation of individuals as care partners within their own health. This theme focuses on demonstrating the pillars of integrated care related to workforce capacity and capability, digital solutions, and aligned payment systems.
