The International Foundation for Integrated Care (IFIC) in partnership with Cúram at Merative, invite you to hear about some of the world’s leading examples and have the opportunity to exchange experiences in order to better understand and prepare for the future together in Finland on Wednesday, 25 October at 12:00pm EET.

The transformation towards a fully person-centered service model requires coordinated health and social outcomes. But what actions are required for progress in Finland? Join us for an interactive session where we’ll explore what the future holds for coordinated health and social protection services in Finland.

The session will address the current needs in Finland, we’ll discuss learnings from other parts of the world, and identify actions for local progress.

We are bringing together experts and leaders across the health and social domains, local government authorities, technology and innovation sectors, and council and board members from
wellbeing areas.

Reflecting on the 9 pillars of integrated care, understanding what can prevent or foster progress, and considering different tactics to realize the expected outcomes of integrated care, raises a variety of questions: are we really aligning around a vision for a more integrated system? Are sources of data and information so fragmented that it is impacting on achieving positive outcomes?  What should be the role of the citizen/patient in the context of a person-centric model?  How can local systems best evidence tangible progress?  The format of the session will invite presentations from leaders in integrated care who will consider some of these questions and more and will be followed by a facilitated discussion including opportunities for smaller breakout groups.
