Shifting to person-centric services with coordinated
health and social outcomes:
Let’s make it happen!
Tuesday, 9 January 2024
12.00pm EST |1.00pm AST | 9.00am PST | 10.00am MST
As someone leading health and social care change in Canada, we would like you to join us for this by invitation only online roundtable discussion organised in partnership with IFIC Canada and in association with Cúram at Merative, to explore the shifting to person-centric services with coordinated health and social outcomes in the Canadian context, taking place on Tuesday, 9 January at 12.00pm EST | 10.00am MST | 1.00pm AST. Please see here the link to confirm your attendance.
The coordination of health and social services is a national priority to address rising costs, changing population needs, long wait times and limited hospital capacity. Transformation requires a model of coordinated health and social outcomes driven by multidisciplinary collaboration to facilitate integrated care policy adoption, implementation, and evaluation to advance integrated people-centred health systems (IPCHSs).
The interactive session will explore what the future holds for coordinated health and social services in Canada, hear learnings from other parts of the world, and discuss progress to date and what future actions are required at the federal, provincial and territory level. We are bringing together experts and leaders across the health and social domains, federal, provincial and territory government authorities, large health providers and networks, the technology and innovation sector, and academia.
