IFIC 9 Pillars of Integrated Care
Shared purpose for integrated care
Shared purpose for integrated care is collectively built and implemented by partners including community sector organisations, public health, primary care, social care, hospitals, long-term care providers, local, regional and national decision-makers.
This is done by aligning services and people to provide people- and community-centred, coordinated, continuous, fair, and locally responsive care.
It leads to shared goals, greater ability to work collaboratively and is linked to desired health outcomes for the local population.
For people and communities
The shared purpose is co-created by the people seeking care and support, people providing care and the wider community. At the local level, everyone understands the shared purpose for integrated care in their area.
For care providers
Care providers understand the local context and build and implement the shared purpose through collaboration across health, social care and other relevant organisations and groups. This includes working with wider partners such as those supporting education, housing, and community groups, and ensuring everyone has a say.
For system-level decision-makers
At the system level, decision-makers from health, social care, and other sectors collaborate based on a shared understanding of common challenges, goals, and the principles of integrated care.
Joint action is aligned with the purpose of partner organisations and local communities and appropriate to the specifics of the local context. This joint action is supported by policy at all the relevant territorial levels.
