IFIC Canada

IFIC Canada, the North American Centre for Integrated Care aims to create a platform to improve the capacity of individuals and organizations in advancing integrated care. IFIC Canada will focus on the principle of people-centred, co-designed care and to achieve our goal of progressing integrated care throughout North America, we are creating a virtual community to bring together all those involved in Integrated Care.

The IFIC Canada community will have the opportunity to develop through the expertise of global leaders in healthcare inclusive of clinicians, policy makers, academics, patients, clients and caregivers.

IFIC Canada also supports the build to the 1st North American Conference on Integrated Care (#NACIC2020), to be held in Toronto, October 5th-7th 2020

The main objectives of IFIC Canada are to:

  • Connect the community to local and global experts, through webinars, video conferences, interviews, international knowledge exchanges and online resources
  • Allow the community to identify the gaps and learning needs of the integrated care community, empowering you to guide the development of the network
  • Use learnings from your colleagues to help you implement your integrated care projects
  • Create opportunities for peer to peer mentorship

IFIC Canada provides an opportunity to connect people together, through the virtual community, creating a space to learn from each other, with the goal of accelerate integrated care.

The virtual community will connect, with a focus on integrated care using the principle of people-centred, co-design to improve practice. Each webinar will use expert knowledge from around the world to provide evidence, insights, peer support and practical tips to support the development of integrated care in the North American context. The learning needs of the network will be identified by the community and incorporated within the virtual sessions.

The virtual community will be facilitated by IFIC Canada and supported by The Change Foundation and the University of Toronto, Dalla Lana School of Public Health.
