This presentation is part of the International Foundation for Integrated Care’s Webinar Series organized in association with Edgehill University. A key principle of integrated care is active patient involvement and empowerment, but in practice, it is often being paid lip service. Until recently, the question of how to involve patients, their families and caregivers as well as their wider communities from the design through to the delivery of health and social services has often been sidelined, even though strong evidence exists for many tools of their positive effect on health outcomes, patient and professional satisfaction and cost effectiveness. In the final webinar, Dr Lourdes Ferrer will give a comprehensive introduction and definition of the different tools and instruments available to involve, support and empower patients, their families and communities to actively participate in the design, delivery and management of health and social services.
