The Central Coast LHD is one of three demonstrator sites of the NSW Health Integrated Care Strategy that took place from July 2014 – June 2017. Central Coast LHD’s approach seeks to provide integrated health and social care to the people of the Central Coast, underpinned by the concepts of inter-organisational partnership, risk stratification and commissioning.
IFIC and CRRMH have been commissioned by Central Coast LHD as part of the NSW Health Integrated Care Strategy to undertake a formative evaluation of the Central Coast LHD’s Integrated Care Programme and the lessons for the future. The three key streams to the Central Coast Integrated Care Program (CCICP) are: Vulnerable Young People; People living with Complex and Chronic Conditions; and Vulnerable Older People. Supporting the three streams is the enablers program comprising business architecture and process analysis, eHealth systems, biostatistics and business intelligence. The evaluation concluded at the end of 2017.
