Over the past five years, Ireland has made significant strides in the national implementation of our Integrated Care Chronic Disease Framework. This initiative involved crafting an evidence-based framework, securing stakeholder buy-in, and obtaining funding, leading us to commence implementation in 2020.
It has been a couple of weeks since NACIC24, and the excitement still lingers. The conference exceeded my expectations in terms of enthusiasm, engagement, sharing, and networking.
Attending the North American Conference on Integrated Care (NACIC24) was an uplifting experience. I had the honour of co-facilitating a 90-minute workshop titled “People Powered Health in Action: Integrating Lived Experience into Organisational Practices,” alongside my co-presenter, Lauryn Seguin from AC/HSO.
Many countries are making the shift towards new models of care, that are person-centered and often community driven. As part of the closing plenary at The North American Conference on Integrated Care (NACIC24) held in Calgary, I was able to offer a glimpse into when I have seen health and social care work within the community.

It’s safe to say that the NACIC24 event in Calgary served as a balm to many a weary person’s soul. While we collectively face difficult problems, this event served as a bright reminder of the hope and optimism available when we come together to share story.

The HealthPathways team had a fantastic time at NACIC24, learning from a diverse range of clinicians, patients, caregivers, researchers, and health system leaders who share a common interest in integrated care. We also had the privilege of sharing our experiences of enabling learning health systems across the global HealthPathways Community.

Making integrated care a reality is one of the most important opportunities in front of us. It is time to improve the experience and outcomes of care, value for money, and to fundamentally shift where and how care is accessed and provided. In many ways, the opportunity in front of us is a universal movement for health system improvement.
